Policy and Procedures

MEDICATION POLICY:

Due to an increasing number of patients canceling or missing their appointments and then calling in for prescription refills, the office policy is now that prescriptions will only be provided at an appointment.

Exceptions:

  1. If, a medically necessary medication change is needed, or a medically necessary dose change is needed.
  2. If we need to reschedule your appointment and your appointment is moved beyond the time your current prescription is active.

We appreciate your assistance in adhering to this policy. We welcome any questions you may have. Adhering to this policy will allow us to continue to provide a high level of patient care.

 CANCELLATION POLICY : 

 NO SHOWS OR CANCELLATIONS BEFORE 24 HOURS IN ADVANCE WILL BE  SUBJECT TO $100.00  FEE.

 IF IT'S POSSIBLE PLEASE CALL US AS SOON AS POSSIBLE TO RESCHEDULE YOUR APPOINTMENT OR AT LEAST WITHIN 48  HOURS TO BEST ACCOMODATE PATIENTS ON A WAITING LIST.

 PLEASE UNSERSTAND THAT REMINDER CALLS ARE PROVIDED AS A COURTESY ONLY AND IF YOU DON'T RECEIVE ONE AND YOU MISS YOUR APPOINTMENT ,YOU ARE STILL RESPONSIBLE FOR THE NO SHOW FEE.

 

EMERGENCY:

 WE WILL DO OUR BEST TO RESPOND TO PHONE CALLS AS SOON AS POSSIBLE, HOWEVER WE DO NOT PROVIDE URGENT, CRISIS OR EMERGENCY SERVICES. PLEASE CONTACT YOUR LOCAL EMERGENCY ROOM, CRISIS INTERVENTION SERVICES OR CALL 911.

 

NEW PATIENTS : 

ON OCCASIONS  NEW PATIENTS ARE SEEN BUT WILL NOT BE ACCEPTED INTO OUR PRACTICE. SEVERAL REASONS MAY PLAY A  ROLE  SUCH AS:  THE NEED FOR A HIGHER LEVEL OF CARE OR OTHER REASONS. THIS WILL BE EXPLAINED TO YOU DURING YOUR VISIT. PLEASE KNOW YOU ARE STILL RESPONSIBLE FOR PAYMENT FOR THIS VISIT.

WE TRY OUR BEST TO SCREEN PATIENTS PRIOR TO YOUR FIRST APPOINTMENT TO MAKE SURE  YOU ARE A GOOD FIT TO OUR PRACTICE, BUT SOMETIMES CERTAIN  INFORMATIONS ARE NOT RELAYED UNTIL OUR FIRST MEETING. IN THE CASE YOU ARE NOT A GOOD FIT TO OUR PRACTICE, WILL EXPLAIN TO YOU THE REASONS FOR NOT TAKING YOUR CASE AND REFERRALS MAY BE GIVEN UPON REQUEST WITH NO GUARANTEE OF PLACEMENT. 

PLEASE UNDERSTAND THAT AN INTAKE VISIT DOES NOT GUARANTEE FURTHER CARE.
